• Illinois Driver’s License or an Illinois State I.D.
• Voter’s Registration Card
• Mortgage Or Lease Agreement
• Public Assistance Medical Card
• Current Property Tax Bill
• Current Payroll Check Stub/ with employer verification form

Students who are registering in the District for the first time must provide the following:

• An Original Birth Certificate
• Physical Examinations and Immunizations(also required for all pre- k, kindergarten and  sixth grade students)
• Dental examinations are required for kindergarten, second and sixth grade students
• New students are required to have an eye examination by a licensed physician or Optometrist.
• All pre-school, fifth, and sixth must show proof of having the Hepatitis B vaccinations
• All kindergarten students must show proof of a varicella vaccination (chicken pox vaccine) prior to registration
• ISBE Transfer Form, (required for all students transferring from an Illinois public school)
• Students transferring from a private school must have withdrawal and grade placement documentation (private school schools will not release information if fees are owed)

• Students who are receiving Special Education Services must present a copy of the current (dated no later than one year) Individualized Education Program (I.E.P.)
• Custody Documentation (if applicable)

All necessary documents must be presented at the time of registration (no exceptions)   *********************************************************************************************************

Textbook rental fees must be paid at the time of registration. Payments will be accepted in form of Cash, Credit card, Debit Card or Money Order. Textbook Rental Fee Payment Information:
